Looking for This Planning Disign Type

I cannot find who created this type of planning disign cover pages and I have found I need a couple of more catagories. My book is ready to be put together but I realized it would be nicer with these catagories.

I believe those are Kellybell's you will have to ask for this on her DISign thread. If someone else doesn't try to match them for you.

I wondered what that font was too. I'm a font freak and I love that one. Does anyone know the name of it?
