Looking for a new design

I am hoping someone can help me with something i have never seen on here before. For my planning binder i am looking for some "header" pages or something like a letterhead page so I can handwrite my notes below. What I have in mind is something that only takes up the top 3 inches of the page with the rest of the page empty. Or it could be just the image (approx 3" x 9") that i could paste onto a clean sheet of paper. I'd like page "toppers" or "headers" to be for:
Dining Reservations
Snacks
Refurbishments
Port Adventures
Fish Extenders
Magic Kingdom Hours
Epcot Hours
Hollywood Studios Hours
Animal Kingdom Hours

I'd love to see these with a Disney flare! In the past I have just done the words with wordart, but I'm sure you creative people on here could do much better than that. Please, Please & Thank You!!!!

Thank you Rustmanfan, but i was thinking of an image together with the wording, something colorful and character-ish that will spread the width of the page. Just about 3" tall x9" wide on the top of a blank page for me to hand-write my notes below. Maybe even as a word-fill? with some characters beside?? The only way i can think to describe it is like a colorful letterhead, disney-style. Thanks in advance!!

Hello! Just an idea...but couldn't you have one of the disigners do the name fills with your headings instead of actual names?
