Link to report missing Photopass photos

Does anyone have a link to the form you use to report missing Photopass photos? I've used it in the past but I can't seem to find it now. The form is mentioned in an FAQ on the WDW site but when you click the link, it takes me to a generic Photopass home page - no form. I e-mailed Disney to ask for a link but that was early last week and I still haven't received a response.

Ignore my post. I was providing a direct link to the missing photos page, but apparently, it has been redirected to the above linked contact-us page. It is often hard to keep up with the changes on the Disney World website lol.

https://mydisneyphotopass.disney.go.com/help-and-support/lost-media is a direct link to the page for reporting lost / missing photos.
