Kindergarten Journal Page Help

I was hoping that someone could help me out. We will be sailing for 18 days and I want to create a journal for my DS that will replicate what he would be doing in school.

A landscape layout with Date and Place lines on upper right of page. Some type of small DCL logo in left top corner.

A large place for him to draw a picture and 3-4 lines at the bottom for him to write.
Would love it if it could say in Disney script "My favorite memory from today is________

If any creative person could help this very challenged mom out that would be great!

Thanks so much I am going to borrow for my grand son who is also in Kindergarten. We are doing 5 days in the world and then a 3 day cruise on the Dream.

Hi I only see the cover pages---was there lined pages too or it that it? I have a kindergartener Id like to make some journal pages for as well and we are doing both Dream and Magic
