Just made a request for private table for dining on Fantasy...

In my experience (twice we were a party of 5) we were seated at our own table. I will point out, however, one time we were a party of 3 and were seated at an 8 top with a party of 5.

If it's important to you to have your own table, I suggest you call DCL and have the request for your own table placed on your reservation.

See, it can happen. :) Princess Shmoo is right...just call DCL and request your own table.
 
We are a party of 5 (2 kids, 3 adults). What type of table do they sit parties of 5 at? I have a feeling we will be using our evening meal as family catch up time so I am leaning towards requesting our own table.
I've been seated at an 8 top twice with parties of 5. My party was 3, others were 5.

It doesn't show on the reservation. You won't know whether you got a table by yourself until you get onboard the ship. At that time you can go to where the dining changes are being handled and ask if you are seated by yourself.

The request isn't noted anywhere on the reservation, either. You have to trust that your TA actually called and had it added to the reservation. You might be able call DCL to "confirm" that your TA put the request there. Ask real nice, as they aren't really supposed to talk directly with you if you used a TA. But, if you say you're just checking to see if the TA did it, they might help you out and check.

Would this be handled at the regular guest services desk or would we need to go to the dining rooms to find someone who could help us with that?

In the Navigator you get in the terminal at check in it will list where/what times the dining changes are being handled. Usually it's in Royal Court, but could vary.

Disney does not, in my experience, list that anywhere on your account that you can access. When you check in at port you get your table assignment, but you still don't know if it is private. To do that you have to (1) show up at dinner and find out or (2) go to the location onboard (ask when you check in for the location, or find it in the information-rich "Navigator" document they give you when you check in) for changing dining assignments as soon as you board and ask them. That location is generally in the main dining room right there in the main lobby (atrium) when you board the ship.
