Jet Blue wheelchair form

As anyone flown JetBlue with a power chair? Any advice? Do I need to fill out the wheelchair information form. Any info appreciated. TIA.
 
If this is it, it might be on your best interest to fill it out since it really covers a lot of questions that would be useful.

I found the form on a google search, but didn't find a link to it in the JetBlue wheelchair information page, so it's probably not required to be filled out.

Sue when you book a flight on JetBlue web site they give you options. It really nice and impressed me how thy did it you could click many options and have you a number to call for further help. They really up their game I and sure all airlines will do it but it reminds me if Disney where you can now book a ha room in their web site just like other guest can book regular rooms. It gives you a chance to book your flight online without having to do extra work besides pick the options that best suit your needs. And some stuff they still ask you to call them but not much.

Your welcome I was going to start a thread when I saw it but had nothing to link and figure no one was wanting to read my explanation.

But if you want to see it you can go in jet blue and pretend to book a flight. On the first page their be some where to click for accessible info and another screen will pop up with all the info. Just about every question someone on here it covered their.

My thought is that with a standard form, they will be able to find the information they need much more easily.
