During the summer, my company sent me a letter stating they had a wrong distribution code on a form that I had included with my 2004 taxes. They said I would have to get in ammended by sometime in August or I could get audited. I emailed my accountant and he said to send him the form.

So, I send him the form. I usually hear from him after a couple of weeks. Didn't hear from him, so I emailed him. No answer. Called him and the number I have gives a weird busy type signal or something. I email him again and again and nothing. It's now almost October.

Should I call the IRS and explain and get a duplicate form from my employer? I have no idea how to get my 1040 ammended. I am not good with this tax stuff. Can I just ammend it myself? I wouldn't even know where to begin since he e-filed.
 
My husband filed an amended return many years ago but I can't tell you how he did it so I'm just going to bump this back to page one for you.

Yes you can amend it yourself. You should have a paper copy of your return. If you do not, you can get one from the IRS by calling 1-800-829-1040. It is actually called a transcript. Also, get a duplicate form from your employer. Get a copy of the original form and the corrected form from your employer. You might need both forms.

To amend your return, you will use Form 1040X. This can be found on the IRS website at WWW.IRS.GOV. Then look for Forms and Publications. Then look for Form 1040X and for Instructions for Form 1040X.

The guy that did your taxes... is he a CPA or does he just do taxes? CPA's can usually be located through your state's State Board of Public Accountancy.

I think I just may call the IRS number included with the letter I got from work. I think I am going to have to start looking for a new accountant.

I am not sure if he is a CPA or not, I will go look up my records and see what he is. He may just be a preparer.
