A "lock-out" is really a one-bedroom and a studio where the connecting door is unlocked forming one 2-bedroom unit. The second bedroom is the same as a studio with the kitchenette and small balcony and a front door to the outside. A dedicated 2 bedroom is always a 2 bedroom. There is no outside door and no kitchenette. I'm pretty sure there is no balcony off the 2nd bedroom but I'm fuzzy on that one.
And yes, they are booked at the time of reservation rather than being just a request.

It depends on the resort. BWV has no dedicated 2 bedrooms, they are all lockoffs. The other WDW resorts have a mix. I'm not sure about HH or VB. You can request one or the other when you make the reservation, they will check availability and let you know.
