How to order custom tiles in grand entrance plaza

All of you who have been to Disneyland in Anaheim have seen the custom engraved bricks in the front. I have searched far and wide and cant seem to find the process in which I should go about ordering these. Does anyone here know how to order these? I would prefer to order it from remote so that I could show it to my wife when we visit Disneyland.

Also, be aware that the installation process takes months. I ordered our brick in mid-June and it's due to be installed in November.

One more thing -- I saved 10% by using my Disney Visa card. Every little bit helps.
