If you & your family are the 1st people to check in for an ADR at 7:15 PM, how do they decide which family to page 1st for the 7:15 PM time slot? Do they do it by how many people are in the ADR, or they do it by who was the 1st family to check in for that time?

It goes by the time of your ADR, the time you check in, and table availability for your party size.

If you are a party of six with a 7:15 ADR who checks in at 7:05, a party of two with a 7:20 ADR who checks in at 7:18 might still be seated first because there is a table for them, but not for your party size. They won't hold back other guests who need a small table while waiting for a large table to be available to another party.

Hope that answers your question.
