We're doing that and the cost was $24 per person. I just had my TA add it to our reservations. I imagine you could call DCL directly and also add it.

You can also purchase a one-way transfer on the day you debark. I've never done this but they always mention it at the debarkation talk. I would imagine that you do it at the Guest Services desk.
