I make some two sided, credit card size with grids on excel cards for each day. I keep them as simple as I can. Here's what I put on them.
Day of week an date
Opening/closing time and park, I already know where I'm going each day.
The parade show times
ADR place and res#/ flight time, hotel res#
The estimated cost of my meal and tip schedule base on those amounts
The weather
On the back I put tips or notes.
Where to watch a parade
Bonus targets on TSMM or Buzz
Photopass poses/locations
Where to sit on rides, back front, left right.
I also make and take a master spread sheet with all the open and close times and show times in case I change things while there. It also has the ADRs in case I loose one of my cards. On this I repeat the tip schedules. It helps me budget because I always take cash to tip so the servers won't get taxed on it.
Just for at home, I make a packing list that I can check off as I get stuff in the suitcase.
