I reserved our ADRs for 4 guests, but there are actually 5 of us. #5 is a 20mth old (at time of arrival). Do I need to change that on my reservations?

Yes. Even babies count as a person for reservation purposes because they take up a seat.

Every single body that will be entering the restaurant needs to be counted on the reservation. So, you are a party of 5, not 4.

Even a newborn baby in arms counts as they'd be included because of the fire codes. Due to humongous fines, WDW restaurants are not going to go risk going against the fire codes. If it's breathing, it counts toward the fire codes. Plus as you said, a 20 month old will be taking up a seat, so they would need a 6 topper instead of a 4 topper (WDW doesn't have 5 topper tables).
