Our extras were similar to most, but I'll list out what I remember:

Floral for MOH and BM and parents
Unity Candle set and petals for the altar
upgraded cake
dinner
wishes cruise on the Grand 1
Marriage license
officiant fee
hair and makeup
bridal attire
rings
attendant and parents gifts
gift for your DH
favors
extra pictures we bought for framing (you get a book of 20 4x6's with the pagckage, we bought a bunch extra, but didn't need the photographer any extended time)

Also don't forget you have your mandatory 4 night Disney hotel stay and we bought the DDE card to get 20% off our sit down meals. It saved us $200 on the reception dinner alone and easily $400+ for the week! The card itself costs $60.
