What about something like this? I wasn't quite sure what you meant by 8.5x11 folded. This image is 8.5x5.5. Please don't quote this image as it takes up linking space. Let me know what you think or any ideas you have for changes.
Here is the crayon box. I have not printed this out so you may have to play with it to get the dimensions perfect. First right click and "save as" a jpeg to your pictures. Open up a Word document and set the margins to .25 and the orientation to landscape. Then insert the image into the Word document. Then click on the picture and go to "format" at the top of word, then "picture" and click on the "size" tab. Change the height to 7.24 and click the tab button on your kepboard and the width will automatically correct itself. Then click "okay". Print on white cardstock and do a trial print using a fast draft and use black ink so that you can cut fold and put the crayons in it to see if the sizing is correct. You could also hold it up to a regular crayon box to see if the dimensions are the same. This is what worked for me...but you may have to tweak it for yours. Also, look at a regular crayon box so that you will know which lines to cut and fold.
