HELP entering Premier Passport Number on Website...no combo works!

I have been trying for an hour- to enter my Premier passport number on the AP website...no combination of numbers work.

Here's what it looks like:
Above the mag strip WTS:CAS### 4/03/2012 ####. (Not sure what that date is....)

Under mag strip
MY NaME
Valid thru: date

Bar code
#### #### #### ####

The website is asking for some weird combo of numbers, but doesn't say which number...HELP please!!!! I am desperate
