Has anyone created their own passport for Epcot?

Have any of you scrapbookers/crafters created a passport for your kids for Epcot? Do you have any pictures to share? Does any have pictures of what the passports look like if you were to purchase one?

I made one, 2 pages for each country. Room for the stamp, a pic of the pavilion and some information about each country, ie population, area, monetary unit, etc. On the other page I have fill in the blank questions, which vary for each pavilion, but include things like: foods tasted, favorite part of pavilion, name in Chinese/Japanese, characters met, and on some i put if they found some of the hidden gems of each pavilion, ie the stave church. Also on each one, I put "I met a CM From" to try to encourage my shy guys to speak with some of the CMs and find out about where they are from.
