Guest Medical Information Form

When I was signing my kids up for the kids clubs online, I noticed the area to fill out a guest medical form that is to be signed by your physician/pediatrician. Does a healthy child need this form signed? Or is it for someone on medications or health concerns?

The medical information form is essentially a request for special services (such as a refrigerator to keep meds cold rather than the standard beverage cooler, a handicapped accessible room, or to have the ship's doctor or nurse administer a medication). It is not needed by anyone not requiring special equipment or services. An adult who takes his/her own medications and does not need to keep them in a refrigerator or a child who takes medications administered by a parent does not need this form.

We submitted the form due to my son's peanut allergy so that they would be prepared for us during meals. He will not be having any meals without us, but I could see it would be necessary if he was eating any meals with the kids club.

Occasionally, the kids club groups went on little "excursions" like to Scoops, so it's not just meals. I don't remember if they had peanuts at Scoops but they do have toppings for the ice cream (by request) so might want to double-check that in case your DS is old enough to be on one of these outings. I know the cast members are very good about such things but it never hurts to be extra aware (as I'm sure you already have to be.)

Thanks for the info! He is old enough, he's 12, so I'm sure he'll be going on an outing such as this. Although he is well aware of what he should eat and not eat, there are hidden dangers. I'll be sure to let him know prior to getting on the ship. Thanks again Eeyorefan12
