2 hours after park opening -- so the actual time may vary but with a 9am opening you can get your next G+ at 11am (or sooner if you used the first). If the park opens at 8am like AK, you can do the first at 7am then another at 10am. If the park doesn't open until 10am like Epcot, you can't grab the next G+ until 12noon.
No. If you scan (and many rides, it's the 2nd scan in LL, not just the first scan), you'll be able to make another. Let's say that scan takes place at 9:40. At that point, you can get another LL starting at 11:40 - OR if your second LL attraction is before 11:40, you can get another upon scanning there. It's not 'both' like you're asking here. I probably just made it more confusing!!
I love the flow chart, but the statement in the Notes that "If you book your first Genie+ after 7am but before Park opening, you will need to wait until 120 minutes after park opening to book your next one" is not true in cases where you tap in before then. The chart has it right; it is the Note that misstates it.

You can make your next one two hours after HS (the park where your first G+ reservation is) opens. That will usually be 11:00. The second G+ reservation can be at any park. If it is for Epcot, you'll get the next available time. If for any other park, you'll get the next available time that is 2:00 or later.
It's tricky to say, because at least on a couple of rides (and I don't remember which ones - sorry) during our trip last week, the second set of scanners were covered up and not in use. Since they typically merge the Genie+ LL queues with the standby queues a little ahead of boarding, that would be your fastest window to grab the next one. Or you can always try to do it in the app to see if it will let you.
