If both the voucher and the AP that the other person had bought a week ago were in the same name, and the voucher was actually issued prior to the purchase of the AP, I can see the refund being given as a reasonable exception to policy. If the voucher and the AP were not in the same name the person who did the refund made an error.
And as far as combining both the voucher and the ticket to an AP, again if the names were the same on both and the ticket was not purchased as part of a package or at a discount I could see it as a valid exception to policy. Again, if the 7-day ticket was either part of a package or bought at a discount the person who did the upgrade made an error in your favor.
