Please post if you figure out how to get this one. After I submitted the form, it said because of high demand, they need you to send an email with your name and address. I looked everywhere I could on their site and couldn't find an email address.

I see what you mean- there is not even something that states -contact us-
on the site--I have a feeling you might get something from them anyway- I filled it our once and I have already gotten -3- brochures from them!

I have filled this out in the past- they dont actually send a free mini bottle- they send you a big folder about their Vodka and a form for a rebate after you purchase a bottle.

DH and I picked up a mini bottle one time at the liquor store just to try it- the bottle is beautiful and the Vodka was really good- drank it straight from the mini! I never drank Vodka staright before- but this was good- it was a little "sweet" if that is a good thing for Vodka- dont know much about it- but I would buy it again!

That makes sense. I have bought wine on-line but I didn't think many states would allow the shipment of liquor.

Yes, it worked fine for me, just print it in color.
When i printed it in black in white they didn't accept it but when i printed it in color it was fine
