I got this coupon in an email from MyPoints but I don't see anything on it that limits its use or has any identifying information whatsoever so let's give it a try:

Print out a coupon to get $5 off when you spend $15 or more at T.G.I. Friday's. Expires 2/29/08.

I did too and if you sign up they send you another coupon for a free appetizer.

How do you get the free sample? Do you need to sign up first?

I did sign up, but it didn't seem to put me on a list for mail or anything (I believe you could opt in or out of that.) But, of course, they need your address to send the samples to.

You just have to enter your info and answer 2 questions about what kind of toothpaste you normally use and what arm and hammer you would like to try. Then click GET SAMPlE.

They did offer another survey and said you would get a coupon if you did it. I did, but didn't see anything come up after. Maybe it would be included w/ the sample?
