Fax form for room request question

Last year I filled out a room request form for Disney and then printed it off and faxed it. I can't remember where that site was. Does anyone know?

It was on the Passperter site but it has been removed. All that remains there is a form you can fill out to generate a telephone script if you call in your requests. It's at http://www.passporter.com/wdw/resortroomrequests.htm if it will help you at all.
