If I am not using the dining plan and I book the F! package at Mama Melrose, will they make my 11 yr old pay the adult rate?
 
Yes the 11 year old will have to pay the adult rate.
 
Your 11yo probably isn't going to get Id'd at the table if they order off the children's menu

But I think you're going to want your 11yo to order off of the adult menu. This July we found that children's menu meals (in addition to having few and awful choices) have really small portion sizes. We're a small family and our kids are off the charts small at theri annual Dr appointments - yet they frequently finished their meals at WDW (especially the 9 and 7yo) and were still hungry, often eventually eating off of my and my wife's plate. Managing this was not fun.

When we go back next year our DD will be 10 so I'm glad she'll be considered a WDW adult for eating purposes.
 
Do you have a set menu (and cost) if you are on the Fantasmic package? Last time I was on the DDP, so I didn't pay attention (and then ended up canceling it anyway) but I was going to pay OOP this time. I thought I just ordered a la carte.

ETA - We are eating at Hollywood Brown Derby, which may be different.
 

You order off of the regular menu and get an app, dinner and desert (even if you are on DDP you get all of these which you would not at a regular TS DDP restaurant). Cost OOP would be fixed price - see below from wdwinfo.com:

Is there a charge for the package?

There is no additional charge for the package itself. However, the package, which includes the Fantasmic! seating pass, MUST be guaranteed at the time of booking with a credit card, and a $10 cancellation fee applies if not cancelled at least 48 hours beforehand.

As of June 2009, you may order from the Brown Derby a la carte menu.

Mama Melrose $32.99 adult/$11.99 child (3-9) not including admission, tax, and gratuity -- Dinner package is price fixe: appetizer, dinner, dessert and non-alcoholic beverage.

Hollywood & Vine $26.99 adult/$13.99 child (3-9) not including admission, tax, and gratuity (no characters). All you care to eat buffet. Choose from a number of selections including; oven-roasted Prime Rib, Citrus Baked Chicken, Pan-seared Salmon, and Stir-Fry Noodles with Sesame. Also includes fresh salads and build your own ice cream/dessert bar. Adult beverages NOT included. Child's meal includes beverage.

It is good to know about the portion sizes. The 11 year old usually orders off the kids menu but finishes everything so the adult portions may be better for her there.
