What are the menu choices when you go to MM for the Fantasmic Dinner Package?

You order from the regular menu. No different with the Fantasmic package.

Yes and Yes, for a while they had a limited menu to choose from for the Fantasmic package, but the have returned to the regular menu. It is similar to using the DDP there as far as your choices from what I understand. Of course Disney holds the right to change anything anytime.
