Our travel agent advised Disney before BUT you can do it at guest services or tell your server. The give her the little cake, a birthday pin with Mickey steering the ship, and that Happy Birthday sign is made of white chocolate. She WOULDN'T share though 
We also went to the Guest Services and they have a number of items you can do. One was the Happy Birthday Banner with Mickey motif that hangs from the ceiling as well as a a Mickey tree and Disney tinsel thing that hangs. They will put it in the room while you're out in the day as a surprise OR you can do it yourself. We did it while we were at CC and then it was there when we came back. THAT is not free however. The cake and pin are.
