If I'm planning a trip in mid-November, can I book the first day of October... 11 months prior or do I need to wait until the same day that I plan to arrive?

If you are planning a Nov 2011 trip you need to book that in Dec 2010. If you are planning on Nov 23, 2011, you book Dec 23, 2010.
