Documentation for DAS

Has anyone requested DAS for a hidden disability (autism in our case) and can tell me what the doctor's letter needs to have? I'm thinking with the translation issue, the simpler, the better but I also don't want to not include something that's needed.

Also, does anyone have experience with USJ and requesting DAS?

We've had no isuees at WDW, DL or DLP but Tokyo seems to me much more difficult. We're going on a weekend in June so it's most likely going to be super crowded but unfortuantely there wasn't another time to go.
 
Do they require documentation now? They didn’t even ask for diagnosis when we got the das a few year ago in Disney World. I would love to hear more recent experiences to prepare for future trips.

American theme parks have always been lax when it comes to documentation.

It's normal for theme parks and other attractions in most of the world to require a disability certificate (or national disability ID in the case of European countries) but since Tokyo Disney has gotten so many Americans without proper disability certificates, they basically let people with official looking doctors letters use DAS.
