Does everyone have to have a "Disney Cruise Line Guest Medical Information" form (signed by a physician) filled out as part of the required documents needed to board the ship?
No, only people who will need special accommodations in order to fully enjoy the cruise. Actually, I was told I needed one for my son (who wears hearing aids and gets 5-day migraines) and was never asked for it. It's probably more important for people who need special diets and/or other medical equipment needs.
