I don't have a middle name on my passport. My driver's license has my middle name (from my birth certificate), my SSN card has my maiden name as my middle name, so not knowing which one to use, I left it blank.
I think that's quite a uniquely American thing, to use your maiden name as a middle name after marriage. If I understand correctly, you were born Jane Mary Smith, then married Bob Jones and became Jane Smith Jones right? I've never seen it anywhere else, definitely not here in Australia, or New Zealand or England. Is it done in Canada as well as the US?
Our passports (Australia) must match your birth certificate + marriage certificate (if applicable). So in the above example, your passport would have to say Jane Mary Jones (if you took your husband's surname).
A friend of mine went to get his birth certificate to apply for a passport, and discovered he'd been spelling his first name incorrectly his whole life, as documented on his birth certificate. He thought it was Roberto, it was actually Robertto. He had to change all his legal documentation (even though he goes by Rob anyway
