Do you have two middle names on your passport?

I don't have a middle name on my passport. My driver's license has my middle name (from my birth certificate), my SSN card has my maiden name as my middle name, so not knowing which one to use, I left it blank.

I think that's quite a uniquely American thing, to use your maiden name as a middle name after marriage. If I understand correctly, you were born Jane Mary Smith, then married Bob Jones and became Jane Smith Jones right? I've never seen it anywhere else, definitely not here in Australia, or New Zealand or England. Is it done in Canada as well as the US?

Our passports (Australia) must match your birth certificate + marriage certificate (if applicable). So in the above example, your passport would have to say Jane Mary Jones (if you took your husband's surname).

A friend of mine went to get his birth certificate to apply for a passport, and discovered he'd been spelling his first name incorrectly his whole life, as documented on his birth certificate. He thought it was Roberto, it was actually Robertto. He had to change all his legal documentation (even though he goes by Rob anyway :) )
 
Since you will only need to show your passport, it's ok if your passport and driver's licenses don't 100% match -- you aren't showing them together, generally. But they should relate to each other. Your social security card and driver's license are more likely to be used together, but again, minor explicable clerical differences are ok.

I have a million names. I was given two middle names at birth (one is my mom's maiden name) and added my husband's name as part of my last name. So -- Sally Jane Brown Smith Jones, call me Sally. One first name, two middle names, two last names (no hyphen, alphabetize with the first last name).

My birth certificate is of course Sally Jane Brown Smith
But my driver's license is Sally JB Smith Jones
and my SS Card is Sally Jane Brown Smith-Jones
and my passport is Sally Jane Brown Smith Jones
(applied for those documents at the same time, with the same backup info. Passport agency got it right, SS Agency assumed I needed a hyphen)
and virtually everywhere else I'm Sally B. Smith Jones
and the email address my work provides is SJSmith@work.com
OH, and when I buy an airline ticket I have to be Sally SmithJones with no space in between last names.

(I didn't make it easy for myself, and should have just made my entire legal name Sally Smith Jones when I got married, but I was young and attached to the names).

I have two middle names on my passport: example: Mary Jane Carson Smith. I kept my maiden name as an additional middle name.
