DISign Help! Can anyone do this???

Hi fellow DISigners!!! I really appreciate all of the work many of the DISers have done for me on these forums and my family and I love the t-shirts we make using the fantastic DISigns. I just have one DISign that I would like to have done to finish off our June 2009 vacation. I need I DVD Case Insert for my vacation video. I have a picture in which I would like to use for the DISign. In the far right corner towards the bottom of the pic I would like to have it say...

Walt Disney World:
Disc One

June 2009

and another DISign with the same pic and the words in the same location saying....

Disc Two

June 2009

If anyone could help me that would be great, because I don't have the talent to do this like you great DISigners! If anyone has any questions I would be happy to answer them. Thank You for any help!!
