Directions to cash out at QR

Can someone please tell me exactly how to cash out at Quick Rewards? I know you click on cashout, but what do you do after that, do you type it in the extra area or what? I would like to cash out since this place actually seems to workout well for me

If you want Paypal, then just write your Paypal email in that box and hit submit.
If you want Disney dollars or gift certificates, then write the gift certificate you want, the amount and your full name and address in that box.

Also, for Disney Dollars you have to have 50-cents for s/h.

So if you want $20.00 in DDs you must have $20.50 in your account.
