Dining Autograph page

I've seen tons of autograph pages for the different Character meals of course. We are using many of them to create an autograph book for the Character Meals we're attending on our upcoming trip.

But I was wondering if someone had or could make a page that I can include for 50's PrimeTime. My girls absolutely love to eat there so we do every time. We've always had great servers who have just put on such a good time for kids I thought it would be great to create a page so they could get the autograph of our waiter/waitress there.

Any ideas? Thanks

OH Thanks so much! That rocks! I know the girls will love to get the signatures from 50's. Not sure how the waitresses will react to being asked, lol. Thanks so much again. IT is fabulous.
