If 2 persons go to a 1TS restaurant and we only want to spend one TS credit, i've read that we can split the food, and additionally for example we can order one extra entree and pay for it, but in this case who decides what entree is included in the plan and what entree we have to pay for it?

Just tell them to charge the more expensive entree to the DDP.
