DDE never received my card

I purchased a DDE card back in Feb. I was told it would take a while for it to arrived, so I asked to have a confirmation letter faxed to me to use for my March trip. That worked just fine. We're planning an Oct trip now, and I just remembered that I never did receive it. Does anyone know what I need to do?

One other question. On that March trip, our family had dinner one night at 'Ohana and I forgot to use the discount. Is there any way to be reimbursed? I'm assuming I'm just out of luck, but thought I'd ask.
 
You need to call Disney Dining 407-566-5858 Option 3 and tell them you never received your cards.

Yeah, you're pretty much out of luck. They won't reimburse you for discounts not taken, especially 3 months later.
