The coverage can be more comprehensive. For example, if you book your own airplane flights, the DCL insurance will not cover the cost of the flights if you need to cancel your trip. Also you can obtain higher coverage limits, add coverage for medical/dental emergency expenses and add a "cancel for any reason" rider. Does it cost less? Usually, but it depends on the policy you select and the issuing insurance company. Some are more, many are less.
I purchase independent insurance for a number of reasons, I don't book my entire trip through Disney as their airfare is normally sky high.
I also can purchase it with pre-existing condition waiver, more comprehensive, and cheaper. Also, I get cash back and I think part of the DCL coverage is that you get a credit to use on a future sailing (in some instances). Also, as I add things to my trip, hotel, transport, an extra day here, airfare, I just call my TA and she takes care of changing my policy and telling me how much more I owe to cover the additional amounts. Also, the coverage offered by the company I use is much better than DCL's for lost luggage, delays, etc.
I did purchase DCL Insurance. I did have to use it because my DS got sick and had to see the ship doctor. I did get total reimbursement of the cost of the visit and medication. We went to WDW after and he had to see a doctor again and I had to go through my regular health insurance. The DCL ins only covered to days on the cruise.
