Cruise form question

Are we really supposed to have that "Guest Medical Information Form" filled out and signed by our doctors or is it just for those with special needs or dietary requests?

In three cruises we've never filled out a Guest Medical Form. Not sure what it's for, but never been asked for it. Maybe it IS for special dietary needs?!?
