Cruise Docs to bring to Port?

Repeat cruisers -- the "signed cruise documents" that I'm supposed to bring with me to the Port are just two pages in length, correct? The first page is just a signed Payment Authorization and the second is the signature of all those who can charge on my KTTW card, correct? Just want to make sure I'm not missing anything. I completed online check-in and printed the document from the DCL webpage.

One more -- if I give my DD permission to charge, does SHE have to sign the form or can I sign since she is a minor? She will charge by simply providing a PIN number, correct? Just like KTTW cards in WDW?
 
you are correct just those two pages, you have to give your DD permission to charge , no pin numbers like in WDW.
 
Optional forms include:
the one that allows minors to embark/disembark with an adult other than those in their assigned stateroom and if you want them to be able to embark/disembark by themselves.

OP - since your family is split across 2 staterooms, you may want/need to complete this form if it's possible the entire family may not be together when debarking at any given port. For example, DS is in your stateroom but he and DW want to head to CC before the rest of you are ready. He'll need this form authorizing him to leave the ship without you (the adult booked in his stateroom).

Charging onboard simply requires the KTTW card, no additional pin.

You could also get those form from guest services, since that is where you turn them in at. I would recommend just filling them out for everyone in your party so you don't have any surprises since you are split across two rooms. I recommend doing it in advance so you don't forget about it and have an issue when trying to get off the ship.

Thank you both for the info. I will fill it out now because the girls will go shopping in Nassau and the boy will be staying on the ship.
