Don't panic. I believe they usually record them by phone number. I know anytime I have called to confirm mine they have always asked for my home phone number first thing. If you are staying onsite I know when you arrive you can have the concierge print out a copy of them for you as well. I would just start off by calling Disney Dining and explain the situation.
