Calling all disigners.... need your help please

I am looking for a template that looks like a recipe card. I want the index card with the little tab on the end. Does anyone have a template I can borrow or can you tell me how to make one? Please... Natalie has one in her photobucket under scrapbooking journal boxes but, I think she is very busy right now.

Here is a link to several "file" templates for file folders that you could use if you have an editing program to delete half of the file folder. If not send me a pm and I can do it for you...just let me know which one you like and send me your email so I can send it to you.
