Broadway Tickets--Add On Incentive

We did an Add-on during the Garden City DTD back in August event and part of the incentive was 2 free tickets to the Lion King. We closed on that add on deal a while ago and have not received our redemption form.

Has anyone received the redemption form yet? Or do you know who I can call other than Member Services??
