We're doing independent excursions on our upcoming cruise to Tortola and St. Thomas--what's the easiest way to get a cab? Are there plenty that are lined up outside the ship terminal? Are they cash-only?

If you google taxis in St.Thomas (Tortola), you can find the current rates for whichever route you need. Taxis are staged near the port entrance. A dispatcher is usually standing nearby to direct people to their transportation. It's easy. I've only ever paid cash.

And know that taxis in St Thomas for specific example don't always look like taxis. The one we were in was a big open air (to the side) jitney sort of vehicle with no seatbelts whatsoever.

Both ports have taxi "dispatchers" just off the dock. This person will direct you to the appropriate private or group safari-bus, depending on your preference and where you're going. The taxis are all there, the dispatcher is there to match you up with the right one.
