If we have points from April 2015 and we bank them in Nov, how long do we have to use them?

Sounds like you have a April UY. If you bank April 2015 points to 2016, they have to be used for a reservation with a check-out date on or before 4/1/2017.

When you bank you move the points into the next UY, when you borrow, you move them to the prior UY. Your UY dates do not change.

They should be, and if you book online, it's very nicely illustrated before and after the points are taken how it's done. But if you do it by phone, it's worth stating it explicitly.

yes, maybe but it's up to you to make sure they use the right ones.
dvc makes mistakes, there your points and your job to make sure it's right.
