Authority Letter to make medical decisions

Hi Folks

We leave in 18 days for 3 weeks of bliss with my two cousins. We have been told that there is a letter that we should download from USA immigration website that my cousins parents can sign to authorise us to be able to make any medical decisions should anything happen to my cousins whilst in Florida. DH and I have both had a look for this tonight with no luck - has anyone used this before or have any idea where I could find it. Fingers (and everything else) crossed that it wont be needed however we dont really want to take that chance.

I'm not sure about the official government version, but there was a thread on the DIS about 18 months ago that had a form posted by a U.S. lady who had taken friends' kids on a trip. Maybe someone with a better memory -- or someone on one of the main U.S. threads -- could direct you.

I took two friends of my eldest son with us to the States last year and got their parents to sign a form a) authorising me to take the out of the the country and b) authorising any necessary medical treatment. The same document also listed their separate insurance policies (we have an annual family deal), any allergies/medical intolerances etc, and the serial numbers of things like their Ipods, cameras, PSPs (If you take teenagers with you, they go equipped with more kit than the military....)

on our last trip we took our eldest DS girlfriend, she was 16 at the time. We just had a letter from her mum giving permission to take her & for any medical treatment she may need. Also took photocopies of the letter.

Luckily we never needed it. Offered to show it at immegration & they said no need but that was 3 years ago.

She also had her own travel insurance.
