For me the attraction where every thing goes wrong is (drumroll) Toy Story Midway Mania (let me explain) If the ride was trying to get to TSMM it would be #1. It starts out as a peaceful(more or less) day at HS and at 9 am
WHAM!!!!!!!! All of a sudden there's people running, trampling babies, getting scolded by CMs, and crying because they didn't get a fastpass and that's just the adults

My husband and I joke about this all the time....everything starts out fine and happy and then, "suddenly something goes terribly worng".....it's the basic plot for most rides, and movies, too!
