Have purchased refurbs several times directly from Apple, with no problems whatsoever.
Every one that we have purchased has been in brand new packaging, and the unit itself has been pristine. If you did not know they were refurbished, you'd have a VERY hard time telling.
We've bought 2 refurbed Nano's. They work perfectly and look brand new. We bought my DH's about 3 years ago and it's still working with no problems. The other one we bought last Christmas and gave it as a gift, and my DD was non the wiser that it was a refurb. I highly recommend it!
I got a square trade warranty on my refurb kindle dx (bought from amazon). They are pretty good about it as long as the item is a factory refurb/recond. You do need to send them a copy of the warranty you get with the item.
Applestore.com has them. Scroll down close to the bottom and look on the left. Sometimes you can also get them at overstock.com and bestbuy.com. They usually go fast especially the newest generations.

We got my daughter a refurbished nano directly through applestore.com a couple years ago. Knock on wood, it's still going strong, no problems at all. I believe the only sign it was refurbished was a sticker on the packaging, which we removed before we gave to her.

Yes. I would say that there are major differences between the fourth gen ipod touch and previous versions. The fourth gen is capable of taking photos and video which the others could not. It also weighs a tad bit less, has an e-book reader, better support for audio books and improved battery life. What really blows most people away is the sharper screen and an apple feature called facetime.
