It's not the $200 for having to see a doctor that you need to worry about (as someone said, probably cheaper outside US). It's the $60,000-$75,000 evacuation if you are very ill or badly injured and have to be evaluated back to US.
I read a story just today about a family supposed to go on Alaska cruise -i forget the line. Bought the airfare through the cruise line and were booked to come in DAY OF CRUISE. They questioned it but cruise line said it would be fine. You guessed it....late plane, they missed the cruise. $60,000. Cruise line so far has offered to reimburse flight but not cruise because they did not have insurance. This should be fun to watch.

a flight from Greensboro that Norwegian Cruise Line booked them on was canceled. Subsequently, the cruise operator kept $58,300 the family had prepaid.
