Anthem BC mammo $50 Visa card

I got a flyer in the mail from Anthem/BC that offers a $50 Visa card if you get a mammogram by Dec 31,2017. All you do is have the Dr sign the attached card (the radiologist signed mine when I had the mammo) and mail it back, no postage required. It's legit and if you belong to Anthem/BC and you're needing a mammo, it's a nice bonus.

ETA: I guess it's actually a $50 Master Card, not Visa.

We have wellness incentives from anthem. I've gotten two $50 gift cards this year (Amazon and Kohl's) for certifying im tobacco free and taking an online health assessment. Once I get a wellness exam I get two more $50 gift cards (have to get a flu shot too but I got that at Target and they gave me a $5 coupon). The program was hidden in our summary of benefits and one of my coworkers who is still on his parents plan found out about it.

We get our summary of benefits for all the plans we choose from emailed to us every year at open enrollment. It was in there toward the end. Also, I found it when I logged in to Anthem, went to menu, and selected the health and wellness center. You may want to poke around in there.

Not every Anthem plan has these and some have other things like gym reimbursement. Also mine is $400 in total gift cards for married couples on the plan.

That's a great incentive! Which Anthem Blue Cross state is your plan from? Is your anthem bc through your work?

This is in California and it's a teacher plan in a high school district. The flyer came in the mail and I almost tossed it!

Thank you for that information! I'll have to look into ours and see if I can view our summary online. We don't have open enrollment and only periodically get a booklet about benefits, but I imagine we could request a copy if the information can't be found online.
