Annual Pass Websiten... HELP!!

I cannot get into the passholder site. I have tried on several occasions and it doesn't work. I do not know what I'm doing wrong. I enter all the information requested and when I hit submit, it just returns me back to the page where I submitted my info - except, now all the number fields are blank. No error message or anything. I don't get it. Anybody have any guesses as to what I'm doing wrong? Thanks.

The site is fairly easy, just enter your name and then pick the annual pass type you bought and enter the codes from the ticket bac as per instructed, click box to save info for re-entering site and then hit submit. If current name and ticket code match, you're in.
